This is the 2010 reprint cover of うずまき（Uzumaki）, originally published in 1998 by Japanese mangaka  伊藤润二(Junji Ito).
Ito is a renowned Japanese horror manga artist, he is known for his works such as Tunnel Vision (トンネル奇譚), Tomie (富江), and Frankenstein (フランケンシュタイン)…

The story of Uzumaki takes place in a town cursed by spirals, following two high school students as they struggle to escape its grip.
The cover was designed by Junji Ito himself. The typography of the hiragana title echoes with the theme of the story: making use of the hiragana script’s simplicity and rounded features, the curves of the characters are cleverly transformed into spirals. Together with the swirling strands of the heroine’s hair, the design visually encloses the two protagonists within the spiral motif in the cover.
