The covers features Janine Lindemulder. An adult film actress in the 90s dressed in a nurse's uniform. 

the cover reflects the playful and irreverent nature of the band's music; the album covers name is a pun on the term “Enemy of the state”. The cover is recognized as one of the most album covers in the pop-punk genre.
